Conditions
Gift Certificates are valid for 12 months from the issue date. Certificates are only accepted by for course Helen Garabedian teaches and can only be redeemed for the value shown on the certificate. Gift Certificates are non-refundable. Certificates are not redeemable for cash and can be up-graded. The original certificate must be presented upon registration or prior to the first class.

Procedure:
Please complete this order form and return with payment to Itsy Bitsy Yoga PO Box 282 Sudbury, MA 01776. The certificate will be issued with 48 hours of payment is receipt and forwarded to the address you have specified. Once the certificate is presented to the recipient they can call Itsy Bitsy Yoga to reigster for their class/es. Recipients have the option to up-grade their certificate if they require a different package.
